 The algorithms and forms of this  guidance document  may be adapted based on the  current practice setting and context. This will serve to inform policy making bodies, but should not be used as basis for denying or approving claims as a result of the non-conformance to the algorithms. 

The recommendations contained should not be interpreted as setting a standard of care. This is not meant to replace the health care worker’s best clinical judgement and shared decision making with the patient or any legally authorized representative (LAR).  Application of a specific therapy or decision must be made ultimately by the health care worker and the patient in light of all the circumstances presented by the individual patient, and the known variability of the medical condition brought about by COVID 19 infection.     

The algorithms and forms reflect the best available information at the time the guidance document was prepared This will be regularly reviewed and revised to reflect new data or information. PAFP and PSHPM do not warrant the accuracy or completeness of the algorithms and forms therefore, the societies assume no responsibility for any injury or damage to persons or property arising from the use of any part of this of these algorithms and forms.
